- Adult Mental Health Teams: Mental Health Teams provide community-based assessment and treatment to adults who are diagnosed with a major mental illness and experiencing significant problems that interfere with their functioning in daily life.
- Child and Youth Mental Health Teams: The Child and Youth Mental Health Team at Three Bridges Community Health Centre (West End) serves children, youth and their families with serious mental health difficulties and/or social, emotional or behavioural disturbances.
- Choice in Support for Independent Living (CSIL): Choice in Support for Independent Living (CSIL) is an alternative method for delivering home support for clients who can direct their care.
- Home Health Dietitians: Home Health Dietitian services range from providing nutrition care to clients recently discharged from hospitals to nutrition counselling for various conditions using individualized diets devised and monitored by a registered dietitian (medical nutrition therapy).
- Nutrition: Dietitians provide information and support related to food and nutrition via consultations, clinics, and health promotion.
- Pediatric and Nursing Support Services: Services and care for children with chronic health, medical complexities and special needs.
- Vision: Public education and referrals to optometry.
