Seniors Remote Monitoring Program

Provides a wearable or in-home device to monitor activty, medication adherence and safety risks.
Seniors Remote Monitoring (SRM) is a supportive program that helps older adults age and remain at home for longer. The program provides peace of mind for both clients and caregivers through 24/7 remote monitoring and easy-to-use technology that is set up in a senior’s home.
The program is provided at no cost to VCH Home Health clients as part of the BC Ministry of Health’s Long-Term Care at Home initiative.The program is currently available to people living in Vancouver, Richmond, North Shore and Sunshine Coast who meet the following criteria:
  • 65+ living at home and needs or will soon need the support of long-term care.
  • A client who is already connected to VCH Home Health Services.
  • Experiencing falls, safety risks, medication challenges, or functional decline.

How the program works:
  • Clients receive in-home and/or wearable devices based on their needs.
  • Devices can monitor activity, safety risks, and medication adherence.
  • When a device detects an issue (such as a fall or abnormal activity levels within the client’s home) an alert is generated and routed to a 24/7 call centre, then addressed according to the senior’s personalized care plan.
  • The Seniors Remote Monitoring clinical team follows up on urgent concerns and collaborates with the VCH Home Health care team.

Technology:
Once enrolled, clients receive devices that help monitor their activity, safety, and health. Clients may receive one or all of these devices depending on their needs:
  • Personal Alert Pendant: wearable device that clients can press to call for help
  • Sensors: Detect movement or inactivity
  • Medication Dispensers: Provides reminders and dispenses medication at the designated time
