Palliative Care (Island Health)

Provides physical, emotional, spiritual, psychological, and practical care for palliative clients, their families, and caregivers before, during and after death.
Hospice palliative care, also called end-of-life care, includes a range of services for dying people of all ages, including children, and their families. By focusing on comfort rather than cure, the goal is to improve quality of life by providing dignity and relief. This is accomplished through various supports for both the dying individual and their family, provided by an individualized care team comprised of physicians, nurses, counsellors, Palliative Care Physicians and Nurse Consultants, and others.

How to Access
  • Physicians and clinicians, please refer to the Physician Information below for private contact information
  • Patients may speak with their primary health care professional, OR by contacting the Home and Community Health Services Intake team, as they will be able to provide information specific to each individual.
Note: Patients must meet the Home Health and Community Health Care eligibility requirements to receive services, which are:
  • Have a life-threatening illness or be facing end of life issues
  • Have a life expectancy of weeks or months rather than years
  • Understand the primary goal of care/treatment is quality of life, not cure of disease
  • Agree to the referral or to consultative support
