Access to hospice is through the FHA Home and Community Care Intake. New clients can contact the Fraser Health Access Line: Home and Community Care (previously called the Home Health Service Line) by calling 1-855-412-2121 (open 7:00 a.m. – 9:00 p.m., 365 days a year) or they can book a telephone appointment by filling out their online form. Current clients can contact their Home Health location. Referring physicians and clinicians can click HERE for the details of the Palliative care consult service.
