Provides client-directed delivery of care with a comprehensive, assertive outreach model for people with multiple complex needs. These services are available in Kelowna and Kamloops
Assertive Community Treatment Teams help you with a team-based care approach. If you are experiencing multiple complex needs and mental illness and/or substance use concerns, our teams provide you with client-directed care, assertive outreach and comprehensive, continuous service delivery.

Services may include: Assistance to find & keep housing, Primary Care, Outreach, Money management & employment, Psychosocial rehabilitation, Court liaison, plus:
  • Screening & assessments
  • Medication administration, review & monitoring
  • Counselling, treatment & recovery plans
  • Crisis intervention
  • Life skills support & education
  • Self-management support
  • Relapse prevention
  • Care coordination & referrals


  • Admission Criteria:

  • Adults (18+)
  • Severe Mental Health Disorder (Schizophrenia, Schizoaffective Disorder), and possible Substance Use issue
  • Inability to participate in traditional office-based or group services
  • Significant functional impairment e.g. Inability to maintain safe and consistent housing
  • Able to function safely in the community with enhanced supports
  • High use of hospital services (e.g. high number of ER visits)
  • Criminal justice involvement (e.g. frequent incarcerations)
  • Complex comorbidities

  • Referral accepted through: Kamloops Community Mental Health & Substance Use Services Central Intake and Kelowna Mental Health & Substance Use and King Street Centre.
